MUSIC FOR YOUNG CHILDREN STUDENT REGISTRATION FORM


ONLINE CLASS TIME:

 SUNSHINE 1 (AGE 3.5-5.5): TUES 5-6PM
Due to the current COVID situation in Ontario and to ensure the safety of all the students, 
Sept to Dec 2021 will remain online.

EARLY BIRD BOOK AND MATERIAL FEE:

 $200.22 (non-refundable)
This book and material fee will be added to your family account upon registering here.

TUITION:

 $27.50/CLASS 

PAYMENT OPTIONS:
MONTHLY (SEPT - MAY) = $110
TRI-MONTHLY (SEP,DEC,MAR) = $330 
ANNUALLY (SEPT)= $990
(Family rate [$26.50/class = $106/month] availablefor second child and up who is ALSO in group lesson.)

 First E TRANSFER will be due by August 1 to secure registration.		 *If for some reasons, you can't attend classes in Sept, the post-dated cheques will be refunded PRIOR TO AUG 1ST!!

To SECURE the registration: 

1. Completed the registration form and agreed to the policy letter,       

2. Paid the book and material fee (via Credit card w/ convenient fee OR E Transfer),

3. Set up your tuition payment.
